Holden Launches Chronic Disease Care Initiative

Governor Bob Holden has joined several of the state’s large health care organizations in announcing the start of a new chronic disease care initiative. The centerpiece of the Governor’s initiative is a recently released plan developed by the State Health Department. The document outlines how health care providers can provide more efficient and patient-focused care to the chronically ill. The initiative was launched at Osage Beach at the state’s first annual conference on chronic disease care management.
